The forms available at the links below are to be used for the generation of new senior electives and for the review of existing senior electives. Guidelines for filling in the elective form are also provided to help departments format the document correctly.
A checklist/routing sheet is attached to help guide departments through the process.
To obtain approval of both new electives and of existing electives that are under review, electives must first pass thorough the Senior Elective Review Committee (a subcommittee of the Medical Curriuclum Committee). Before considering an elective, the Senior Elective review Committee requires that:

1. Electives are in the proper format and adhere to the guidelines. |

2. Any faculty listed on the elective must be confirmed by the department and the campus Dean. |
Corrections must be made by the department and approved by the Campus Dean before receiving MCC approval.
