The NDPERS Diabetes Management Program

CHPPR was awarded a three-year contract to evaluate the NDPERS Diabetes Management Program which began in July 2008. The NDPERS Diabetes Management Program is provided by pharmacists and other health professionals throughout the state and focuses on helping members better understand and control this harmful disease. The goal of the program is to provide an integral component of diabetes care using the National Standards for Diabetes Self-Management Education. CHPPR’s evaluation of this program will include longitudinal cost analyses using multilevel or hierarchical modeling to control for the effects of patient age and the intervention provider. These analyses will contrast costs of those patients in the program with those eligible for, but not enrolled in, the program. Return on investment ratios will also be calculated to assess the program’s cost effectiveness. Finally, secondary multilevel analyses of the NDPERS DMP participant data will determine which factors are most predictive of successful clinical and cost outcomes.
